Disclosure
By clicking “Get Started”, the Authorized Pharmacy Representative hereby authorizes Square to disclose to Cardinal Health, Inc. onboarding status, transactions history, including sales, refund and fee amounts maintained by Square within your Square Account created via this onboarding process.
Additionally, the Authorized Pharmacy Representative agrees to send Square additional compliance documentation as required for high risk onboarding if 20% or more of your transactions are processed Card Not Present. In the future, if Pharmacy surpasses the 20% Card Not Present threshold, additional compliance documentation will be required. Pharmacy also acknowledges that Square reserves the right to deactivate any account found to process over 20% Card Not Present without receiving prior written approval by Square.
