New Patient - Hearing Test & Hearing Aid Consultation
For patient's that have never been seen in our office before and are interested in a hearing test and hearing aid consultation. *If you have Medicare Insurance a referral will be required from your primary care physician for a hearing evaluation. **There is a $50 appointment fee for a Hearing Aid Consultation, some insurances cover this charge. ***If your insurance has hearing aid coverage they usually require prior authorization before your appointment. If this applies to you please schedule your appointment at least 5 business days out to allow us time to receive that authorization.

This appointment also includes time to discuss test results and treatment options if a hearing loss is present but may not allow for enough time for a hearing aid consultation. Testing = $75 to $150, will bill to insurance if possible. *If you have Medicare Insurance a referral will be required from your primary care physician for a hearing evaluation.

Hearing Aid Evaluation
My hearing has been tested in the last 6 months and I am interested in purchasing hearing aids. *Please bring previous hearing test with you to your appointment or contact the office and have it faxed to us at 833-277-9264 **Some insurances may cover this service but may require prior authorization so please schedule the appointment at least 5 days out to allow us time to request and receive that authorization.
